-->> Registration Information Form – Completed and saved online
-->> HYF Waiver Form and Equipment Form – Signed at registration
-->> Your player - Players need to be weighed and fitted for their Jerseys
-->> A COPY of the players birth certificate
-->> A COPY of your health insurance card
-->> Cash, Check, Visa and MasterCard are acceptable forms of payment
-->> $300 equipment deposit check post dated 12/1/2012 (will be returned when equipment is turned in)
-->> Jersey numbers picked at early registration only!
